What You Need for Your Intake Appointment

Thank you for choosing Jan Dils, Attorneys at Law, to represent your Social Security case! We’re here to help, but we need some information from you in order to get started.

First, identify which type of intake appointment you have (Online Application, Pending or Appeals/Hearing Requests). Then, see below for what we will need from you to have a successful intake call.

Online Applications

Please have the following information ready for your intake call:

  • A list of your current medical providers, including their name and address
  • A list of medical providers you saw during the five years before you stopped working, including their name and address
  • A list of current medications you are taking
  • Dates for any upcoming medical appointments
  • Your last FIVE (5) jobs, including company, job title, start date and end date (dates can be estimated, but please try to be as accurate as possible)
  • Your earnings this year and last year, if you had any, including any income you made through self-employment
  • If you are currently married, your spouse’s name, date of birth, social security number, marriage date and the city/state where you got married
  • If you have a previous marriage that lasted longer than 10 years, or if your spouse passed away, we will need your former spouse’s name, date of birth, social security number, marriage date, city/state where you got married, and the date of divorce or date of death of your spouse
  • Banking information including routing and account numbers (This information will be used for direct deposit. Providing it is optional. If you choose not to provide this information with your application, the SSA will work with you to find another way to get your payment to you)

Pending Claims

Please have the following information ready for your intake call:

  • A list of your current medical providers, including their name and address
  • A list of current medications you are taking
  • Dates for any upcoming medical appointments
  • Your last FIVE (5) jobs, including company, job title, start date and end date (dates can be estimated, but please try to be as accurate as possible)

Appeals/Hearing Requests

Please have the following information ready for your intake call:

  • A list of your current medical providers, including their name and address
  • A list of current medications you are taking
  • Dates for any upcoming medical appointments
  • Your last FIVE (5) jobs, including company, job title, start date and end date (dates can be estimated, but please try to be as accurate as possible)

We encourage you to use electronic options for filing your paperwork, because it can make the process quicker and easier. We will work with you to find the best filing option for your situation.

What to Expect During Your Intake Call

On the day of your intake appointment, we will call you at the scheduled time (If you can no longer make your appointment time, that’s ok! Please get in touch with us as soon as possible and we will reschedule).

During the intake call, we will verify all contact and demographic information so your legal paperwork is printed correctly. We will gather all the details that your case manager will need to begin working on your case.

At times, the intake appointment may seem repetitive. We will ask you to confirm some information we have from your initial call. This is because we want to ensure every detail is correct and accurate in your claim, and that we include any information that may help your case. Thank you for your patience as we work together to complete your paperwork!

If you do not have a currently pending claim with the SSA, we will complete your application or appeal with you during the intake call. If possible, we will file an electronic application/appeal on the SSA Website while you are on the phone. Otherwise, we will complete a paper application for you if there are issues with the SSA website or if you have special circumstances that require it.

If there is no legal action of appeal or application that needs to be filed immediately on your claim, we will confirm the details of your case with the SSA. This shows us that your application has in fact already been filed and your claim is actively pending with their office. This ensures that the SSA is working on your case, so we can fight for your benefits.

We will also review your authorized contacts – people you identify that we can speak to about your case in the event of an emergency. We will review expectations for our client-attorney relationship, and review our contingency fee agreement. As a reminder, we will not ask for any payment upfront! The contingency fee agreement means we only get paid if you win your case, and our fee comes out of your backpay.
