Debunking Forms: The Application for Disability Insurance Benefits

Today, we’re finishing our series on forms with the Application for Disability Insurance Benefits (Form SSA-16-F6). This form is used if you are applying for Social Security Disability Insurance Benefits and is one of three forms that must be completed when you are initially applying for benefits. You will also need the Disability Report and the Authorization to Disclose Information to the Social Security Administration.

First and foremost, it’s important to thoughtfully consider your answers before filling out this form. If you do not know an answer, writing “I don’t know” is better than not writing anything. Similarly, if a question does not apply for your situation, write “N/A” or “not applicable” instead of leaving it blank.

For this particular form, you will need to supply basic details about yourself such as your marital status and Social Security Number (SSN), as well as detailed information about your disability. The Social Security Administration (SSA) will want to know how the ailment affects your work, if it’s related to your work, the dates you have served in the military, any previous benefits you have received, your work history, and more information to assess your case.
